
A class action settlement involving Global Fitness Holdings, LLC (formerly d/b/a Urban Active Fitness) ("Urban Active") was preliminarily approved in the United States District Court for the Southern District of Ohio on September 30, 2013.
Under the proposed settlement, individuals who signed a gym membership contract or personal training contract with Urban Active during the period of January 1, 2006, to October 26, 2012 (the "Class Period"), are Class Members and are eligible to receive money under the settlement. The settlement also provides for the three subclasses described in the boxes below.

Paid a $15 Facility Improvement Fee, Club Administrative Fee, or any other biannual $15 fee to Urban Active on or between April 1, 2009, and October 26, 2012.
Cancelled a gym membership contract with Urban Active on or between January 1, 2006, and October 26, 2012.
Cancelled a personal training contract with Urban Active on or between January 1, 2006, and October 26, 2012.
To qualify for payment, Class and Subclass Members must complete and return a Claim Form on or before December 30, 2013. Claims can be submitted by mail or filed online through this website.
A Postcard Notice and email Notice explaining the Lawsuit and settlement was sent on October 30, 2013, to the individuals listed in Urban Active’s records as having signed a gym membership or personal training contract with Urban Active during the Class Period. If you did not receive a Postcard Notice or email Notice, but meet the requirements of the Class/Subclasses defined above, then you may still be eligible for a Claim Award (monetary compensation) and may be bound by the terms of the settlement.
A Fairness Hearing has been scheduled with the Court on February 13, 2014, at 10:00am in Courtroom 228 of the United States District Court for the Southern District of Ohio, Eastern Division, 85 Marconi Blvd., Columbus, Ohio. At that hearing, the Court will determine if the settlement is fair, reasonable, and in the best interests of the Class and Subclasses. Claim Awards (monetary compensation) cannot be distributed to Class and Subclass Members until after the Court grants final settlement approval and any appeals are resolved.
